- Routine Home Care (in a Home, Residential Hospice Unit, Nursing Home, Hospital, Private Care/Group Homes, and/or assisted living facilities)
- Continuous Care (Home, Residential Hospice Unit, Private Care/Group Homes, and/or assisted living facilities)
- General Inpatient Care (Nursing Home or Hospital)
- Respite Care (Nursing Home only)
